Looks like the clutch was engaged (for some reason) and at that point the brake would not hold it. Once it started.....it's all over....

Air brakes are for train control, not to hold railways vehicles in place (the air bleeds off). Mechanical (hand) brakes applied to railway vehicles will hold that railway vehicle in place. Rules dictate how many handbrakes must be applied to certain amounts of railway vehicles while also considering...

Could be a few things, either equipment coming (more "lend-lease" CH-47Ds) or going (attrition replacement and forces boost to the sandbox).
