Given that it was a fatal accident that happened to an US airline in US soil (well, sea) and it involves the FAA and NTSB, it is strange that it didn't have any repercussion there.
Speculation in different fora has it as fuel exhaustion.

Does Jet-A float over 100LL? Because the pane flew a good while at constant speed and altitude before starting to descend.Fixed.Speculation in this forum has it as mis-fueling with Jet-A
Anyway, since the FO is alive and stable, I think we will know.

Both being petroleum distillates, I'd expect they'd probably just mix, and form something you definitely would not want to put on your french fries.

Gratuitous speculation by me, and fuel starvation and wrong engine shutdown and meteor strike and a number of other potential causes have not been discounted.
