Ionian Sea Cyclone crash

Last year a Canadian helicopter faceplanted in the ocean (we don't seem to have had a thread about it) and this week the report is out, and it seems the flight control software is the chief cause. I am not_surprised but it looks like it's fairly glaring: manual control inputs without disconnecting the autopilot confuse it.

Pilots good...procedures, training, screening, regulations, oversight and less automation.
