2. Not that I can recall, at least not when both were stationary (see next answer).
3. I don't believe I have actually ever seen it written anywhere, but generally accepted practice appears to be that one does not taxi into position until the preceding aircraft has started moving. At any rate, I don't see what the UA crew's hurry was, but whatever.

Based on some documentaries I've seen on the intarwebs, it may have had to do with snacks.At any rate, I don't see what the UA crew's hurry was, but whatever.
