1. Do you find this to be:

A. Ordinary and of little significance.
B. A bit unusual
C. Very unusual
D. Extremely unusual and bad practice and worthy of scorn from ass-hat outsiders.

2. How often do you see two airliners on the runway at the same time, waiting for takeoff?

3. What do you consider a proper spacing for this situation.

2. Not that I can recall, at least not when both were stationary (see next answer).
3. I don't believe I have actually ever seen it written anywhere, but generally accepted practice appears to be that one does not taxi into position until the preceding aircraft has started moving. At any rate, I don't see what the UA crew's hurry was, but whatever.
This was not just garden-variety friendly fire.

Footnote: Does your FCOM really have a chart of exhaust velocity vs distance…with “hard” numbers?

Based on some documentaries I've seen on the intarwebs, it may have had to do with snacks.
